Application forms can be printed at interview or printed online at www.passports.gov.au.

The following documents must be produced at interview/lodgement:
1. current passport
2. Two quality photos

For a child (aged 16-17) passport application, the child does not require to be present at interview/lodgement. The parent or someone with parental responsibility may lodge the application.

The following documents must be presented at interview/lodgement of the application:
1. Child's original birth certificate and
2. Citizenship certificate (if applicable)
2. Child's current passport (if applicable)
3. Lodging parent's ID (passport)
4.Non-Lodgng parent's ID (Showing Current residential address)
5. Two quality photos - one must be signed at the back by the guarantor.

Fee, can be paid by vatu cash, bank transfer and credit cards.
